PLEASE NOTE: once you make an appointment with us, we will email your registration login information to you. Follow the instructions in the email to log in to your chart. Keep in mind, the user name and password are case sensitive.

  • We verify your insurance benefits and coverage before any procedure.
  • We will estimate the portion your insurance should pay.
  • We will file your insurance claims electronically for you.

The safety and well-being of our patients, community, and staff are always our number one priority.

As the current COVID-19 situation is changing rapidly, we want to reassure you we are adhering to the most up-to-date screening guidelines and highest standards from the Centers for Disease Control (CDC).

To ensure your safety, we have instituted the following protocols:

  • COVID Protocols

    • We will only allow one patient at a time in our waiting room and treatment rooms to respect social distancing requirements.

    • We will ask COVID-19 screening questions when making your appointment and the day before your scheduled appointment.

    • Please come alone unless the patient is a minor or you are having sedation

    • Everything you encounter, such as chairs, door handles, pens, keyboards, and counters are sanitized before and after your use.

    • We have hand sanitizer gel and spray throughout the office for your use.
